Getting Started
If you would like to schedule an appointment, please follow these steps:
- Make sure that your insurance is accepted or that you understand the cost and payment options.
- Schedule an appointment with me by phone (303) 981-5586 or email. In your email – or if you need to leave a voice message – please include your name and phone number as well as the times that you may be available to meet (generally), and what services you are requesting.
- Before you attend your first appointment, please fill out the Initial Intake Form. For your protection, all information is encrypted during transmission to my office.
